With just three simple steps, patients can now request their E-LOA online and experience a more efficient approval process:
- SCAN the QR CODE or go to bit.ly/3Fk2ZR8 to ACCESS the E-LOA Request Form.
- FILL OUT the required information, UPLOAD the necessary documents, and CLICK Submit.
- WAIT for an email confirmation and PRESENT the approved E-LOA to the HMO Staff at the HMO Lounge in GoodSam Medical Center.
Reminder: All E-LOA or online applications must be done at least a day before your preferred schedule to ensure timely processing.
